Notice of Privacy Practices
Effective Date: 16th August 2026. Your information. Your rights. Our responsibilities. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Your Rights
You have the right, as applicable, to: get an electronic or paper copy of your medical record and other health information we maintain about you; ask us to correct health information you believe is inaccurate or incomplete; request that we communicate with you in a particular way or at a particular location; ask us to limit certain uses or disclosures of your health information; request an accounting of certain disclosures of your health information; get a paper copy of this Notice at any time; choose someone legally authorized to act on your behalf; and file a complaint if you believe your privacy rights have been violated.
If you pay for a healthcare item or service out of pocket in full, you may ask us not to disclose information about that item or service to your health plan for payment or healthcare operations. We will honor the request unless disclosure is required by law. Practistat will not retaliate against you for exercising your privacy rights or filing a complaint.
Your Choices
In certain circumstances, you may tell us your preferences about sharing relevant health information with family members, close friends, caregivers, or others involved in your care or payment for your care. If you are unable to communicate your preference, we may share relevant information when permitted by law and when doing so is in your best interest.
For uses and disclosures that require your authorization, we will obtain your written permission. You may revoke an authorization in writing as permitted by law.
How We May Use and Share Your Health Information
Treatment: We may use and share your health information with healthcare professionals and organizations involved in providing or coordinating your care.
Payment: We may use and disclose your health information to bill and obtain payment for healthcare services.
Healthcare Operations: We may use and disclose your health information to operate Practistat, coordinate services, improve quality, conduct compliance activities, and perform other healthcare operations permitted by law.
Other Uses and Disclosures
We may also use or disclose health information as permitted or required by law for public health and safety activities, health oversight, workers’ compensation, certain law-enforcement or governmental requests, judicial or administrative proceedings, medical examiner or funeral director activities, organ and tissue donation when applicable, and research when applicable legal requirements are satisfied.
Substance Use Disorder Records
If Practistat creates, receives, or maintains substance use disorder patient records protected by 42 CFR Part 2, additional confidentiality protections may apply. Such records may not be used or disclosed in investigations or proceedings against a patient except as permitted by applicable law. Additional consent and redisclosure requirements may apply.
Our Responsibilities
Maintain the privacy and security of protected health information as required by law. Provide notice of our legal duties and privacy practices. Follow the duties and privacy practices described in the Notice currently in effect. Notify affected individuals when required following a breach of unsecured protected health information. Obtain written authorization when authorization is required by law.
Changes to This Notice
We may change the terms of this Notice and our privacy practices as permitted by law. The current Notice will be available upon request and posted on our website as required.
